About the CFPHD
The Central Florida Partnership on Health Disparities is a broad-based coalition of individuals and organizations in Central Florida that serve and represent ethnic and racial groups in Orange, Osceola, Seminole and Brevard counties. The CFPHD functions as a collaborative vehicle for community partners to work together to eliminate health disparities, rather than duplicate efforts and compete with one another. |
History
The CFPHD, which emerged from the Health Resources Services Adminstration (HRSA) initiative: 100% Access and Zero Percent Health Dispartities, was launched in 2004. The development of the partnership was guided by "Mobilizing for Action Through Planning and Partnerships" (MAPP), a collaborative and proven public health model that helped the partners define a vision, set measurable outcomes, identify existing minority health initiatives and programs, identify community barriers to health and develop action plans to reduce health disparities. Today, members of the CFPHD, such as the Health Council of East Central Florida, Inc., contribute heavily towards facilitating the MAPP process in the region.
